A defined review model

At initiation, the parties agree which questions require medical review, who supplies the necessary information, how urgent matters are identified, and where decisions are recorded. Response expectations are based on protocol risk, enrollment activity, geography, and the contracted coverage model.

This creates a repeatable path from question to review, decision, communication, and closure. It also helps prevent important medical issues from being managed only through informal email threads.

Interfaces and accountability

4Cs can coordinate with the sponsor medical lead, pharmacovigilance function, CRO medical monitor, clinical operations team, and other designated parties. Responsibilities are documented so that medical oversight complements, rather than duplicates, existing safety and operational processes.

Operating boundaries

The engagement does not replace investigator medical judgment, emergency medical care, pharmacovigilance case processing, or sponsor regulatory accountability. Continuous or after-hours coverage is provided only when expressly scoped, resourced, and documented.
