Oversight capacity is not the same as headcount
A small sponsor does not need to reproduce every CRO function internally. It does need enough qualified capacity to understand important activity, evaluate risk, make sponsor decisions, and verify follow-through.
Build the minimum effective system
- Name accountable sponsor decision-makers
- Define CRO and vendor interfaces
- Establish a risk-based governance cadence
- Maintain one reliable set of risks, issues, actions, and decisions
- Set escalation thresholds before a crisis
- Review evidence around participant protection, data reliability, and major milestones
Add capacity where judgment is needed
Embedded support can be useful when a sponsor needs senior clinical operations or medical perspective but does not yet need a permanent full-time structure. The scope should be explicit, integrated with existing providers, and designed for continuity.
Avoid governance overload
Lean teams are especially vulnerable to duplicate trackers and low-value meetings. Use a small number of artifacts that produce visibility and decisions. Expand the cadence temporarily when enrollment, safety signals, vendor recovery, database milestones, or inspection risk justify it.
